2010 Cal-ED Conference : Award Nomination Form

(one submission per nomination)

Award Nominee Applications are due by February 15th, 2010

The General Membership may select any or all of the Awards below. The only exception is the Student Scholarship Award which must be submitted by University Department Chairs. Additionally, the person who is nominating must remember to scroll all the way down to include a written description of the person being nominated and to input her/his own information in case the Awards Chair needs to contact you for more information.

Please submit three letters of recommendation for the nominee(s) with your contact information plus a 2″ x 2 1/2″ photo of your nominee to Dave Smith

For each person nominated, please type a short biography, no more than one page. This biography should be a little about this person and why s/he should be an Award recipient for the current academic year (2008-2009).
